Academic Early Warning Form

Early Warning is an effort to increase student academic success and improve student retention. Please report any students who are having difficulty with your course. Once you report those students, an SPS Academic Advisor will contact the student to assist the student with identifying resources and developing a plan of action. The SPS Academic Advisor may also contact you at a later date to find out if there has been improvement with the student.

NB: Please continue to speak with the student about their standing in your course.  Reporting does not eliminate that responsibility.

After you report this information, an SPS Academic Advisor will follow up with you via email to find out if there has been improvement with the student.
