Documentation Guidelines

Please read this information regarding the documentation requirements of both physical and learning disabilities. Students or health care professionals who have questions not covered in the guidelines should contact the Academic Services Center at (202) 884-9681

To initiate services and request accommodations, students should make an appointment with Melissa Earnest (202-884-9681), Library 103, before the semester begins or as soon as possible thereafter.

The Director will consult with students to determine the appropriate accommodations and/or modifications needed.

The Director will work with students to develop an individual learning plan that will enable students to reach their academic potential.

Students must complete a Request for Letter to Professors form each semester in accordance with confidentiality regulations.

Students with learning disabilities and other disabilities are encouraged to take advantage of the tutoring services offered by the Academic Services Center and services offered by the Health and Wellness Center.
